Long term health conditions

Advice for health professionals to support people with long term health conditions to be active.

One in three of us in England live with a health condition. This group is twice as likely to be amongst the least physically active, yet we know that being active can help to manage these health conditions and increase our quality and length of life. Health and care professionals can play a crucial role in supporting people living with long-term health conditions to move more in a way that works for them.

We Are Undefeatable

We are Undefeatable is a national campaign to support people who live with one or more long-term health conditions. It aims to help those with conditions such as diabetes, cancer, arthritis and Parkinson’s to build physical activity into their lives. The website is a great resource for both people living with a long term health condition and health and care professionals working with them. Bridging the Gap -Understanding How to Support People With Long-term Health Conditions to Become More Physically Active


Discover the findings and insight from We Are Undefeatable’s Big Talk: Public Consultation. This discussion gathered input from over 2,000 people, including those living with health conditions, their carers, friends and families, plus those with a professional interest in this area.

Movement for All Programme - Resource Pack

Through the Movement for All programme, The Richmond Group of Charities and their partners have gathered insights into the common barriers preventing people with (multiple) long term health conditions from being active. They have put together a resource pack for health and care organisations to support people with long term health conditions to be more active.

This pack is aimed at health and care organisations, such as those working in partnership as Integrated Care Systems, and provides:

• Four ways your organisation can make a difference and help people with long term health conditions be more active.

• Information about why it’s important to support people with long term health conditions to increase their physical activity – including your own workforce.

• Links to resources and case studies with more information and inspiration from good practice
